A case of isolated capitellum fracture fixed with headless screws

Abstract


Aim The purpose of the study was to evaluate the clinical, radiographic, and functional outcome following open reduction and internal fixation of type-1 capitellum fracture with Herbert screw. Material and Methods A case of           12-year-old girl with a displaced fracture of the capitellum of the right elbow, a typical Hahn-Steinthal or Type 1 fracture was treated with open reduction and internal fixation through kochers approach using Herbert screws inserted  anteroposteriorly. Patient had no associated injuries or neurovascular compromise. AP and lateral views were taken. CT scan helps to find any associated bony injuries. Elbow pain, range of motion, stability, and function were evaluated using the Mayo Elbow Performance Index. Results The follow-up period was 1 year. At the fourth month of follow-up, the fracture was considered completely healed and patient achieved full range of motion. No evidence of avascular necrosis, heterotrophic ossification, physeal arrest or angular deformity seen. final follow up X ray show anatomic bone union. The Mayo Elbow Performance Index score was 100, indicating excellent outcome Conclusion A displaced Hahn- Steinthal type 1 capitellum fracture must be anatomically reduced to restore articular congruity and to minimize potential disablement. We conclude that herbert screw fixation is a reliablemethod of treatment because it enables a good interfragmentary compression, earlier mobilization, faster functional elbow recovery .Headless screws provide more fixation stability than cancellous lag screws do.
