Intestinal Obstruction Due to Fibromuscular Dysplasia
Abstract
INTRODUCTION : We describe a rare case of small bowel infarction caused by fibro muscular dysplasia with a good prognosis CASE PRESENTATION : We present a case, where an elderly male presented with cardinal symptoms and signs of intestinal obstruction. Necessary investigations were done which concluded small bowel infarction. Exploratory laparotomy revealed jejunal gangrene. Resection anastamosis done. Histopathology was suggestive of Fibro muscular dysplasia (FMD). Post operative period was uneventful and he was discharged with antiplatelets with regular follow up. Disease specific investigations to rule out cerebral vessel involvement were all negative. Thus the diagnosis of ischemic bowel disease produced probably by Fibro muscular dysplasia was made. CONCLUSION : This case illustrated the importance of consideration of mesentric artery FMD and cause specific treatment should be tailored to any case of intestinal obstruction to prevent recurrences. Owing to its rarity, mesenteric vessel FMD still eludes the surgeon in making a diagnosis. Hence a high degree of suspicion is important for appropriate management.
